Dudley Health & Well-being Partnership Questionnaire

Please let us have your views about what we need to do to improve local services.  We are looking for big ideas and big priorities which will do one or more of the following:

  • achieve better health and social care outcomes
  • improve service quality
  • increasing patient/user involvement and satisfaction
  • use our resources for maximum impact

Please try to be as specific as possible and give examples – eg. if you think that services need to be more flexible, say why and how, and provide examples.

1) What works well?
2) Which groups are getting a raw deal?
3) What help do the groups who are getting a raw deal want and need?
4) How can things be changed for the better?
5) How can our local services fit better with the way people live their lives?
6) How can local people be helped to help themselves?
7) From you experience do Dudley people miss out on services that are provide by Voluntary Sector Organisations in other compared with other parts of the West Midlands/Country?
8) If you had to pick 5 big improvements to the way we fund and organise health and social care, what would they be?
9) If we had to stop doing some things in order to fund what people need most or what is most effective, what would you stop?

Thank you for your time and your ideas. It would help us to know who you work for or which organisation you are affiliated with and whether you would be interested in being involved in the later stages of this work. Please provide your details at the end of the questionnaire.
