Instrumental Tuition Application Form

Anyone wishing to have instrumental and/or vocal lessons with DPA need to complete and return the attached form.

On receipt of the form, DPA staff will process your application and tuition will normally commence at the start of the following term. In order that time tabling can be scheduled effectively, the deadline for applications is typically a few weeks before the end of term. Please contact DPA on 01384 813865 for current deadlines and costs.

All fees will be returned in the event that we are unable to provide tuition (e.g. no suitable teaching group, no instrument available).
