All DPA Performance Group participants have to complete and return the attached form in order to be eligible to perform with DPA at the many concerts, venues and festivals.
The form has to be completed and signed by the child's parent or guardian and gives their permission for the child to take part in DPA events. These forms are renewed annually.
This form should be completed and returned to the performance group director.
