Community Team Learning Disability

 
The Community Learning Disability Teams (or CTLDs for short) are made up of Social Workers and Nurses.  Their job is to assess people who ask for services and to arrange these services.
 
The Social Worker or Nurse will discuss with you  and your carers the help you may need – this is called an assessment.
 
If your assessment shows that you need help, we will arrange this.  We will give you a copy of your care plan which sets out the details of the services you will receive.
 
Your needs may be met by using Direct Payments, Independent Living Fund monies, by services from the voluntary or independent sector or by the Directorate of Adult, Commmunity and Housing Services. 
 
We will review the service with you regularly to make sure it is still meeting your needs
 
The Social Worker will also offer a carers’ assessment to identify what problems your carer may have in their caring role.
