The fostering team have created an online form specifically for children in foster care as a way of finding out what your thoughts and feelings are about your placement and the care you receive. We would like the you to complete this form so that your views can be heard.
The form is secure and confidential so your answers are private unless you choose to tell someone. Once completed the form will be automatically sent to a special fostering inbox which only fostering team members can access.
The form is very short and comprises of five sections; your views, how I feel, what I want and what I think followed by the opportunity to review and then send.
It can be filled in whenever you want and as many times as you would like to.
