Black Country Sustainability and Transformation Plan

For the vast majority who may not be familiar with STPs; Every health and social care system in England must produce a Sustainability and Transformation Plan (STP) showing how local services will evolve and become sustainable over the next five years, in order to best meet the needs of local communities. (More information can be viewed here on NHS England’s website.)

The All Together Better partnership brings together all the health and social care organisations across our area to review and shape future health and social care services in Dudley. Integration & Better Care Fund

Our Integration and Better Care Fund Plan for 2017-19, which creates a £148m pooled commissioning budget over the 2 years of the plan, has now received the highest level of assurance from NHS England and the Local Government Association. The plan describes how health and social care integration will support the transition to a new model of care in Dudley and is fully aligned to our shared plans for a Multispecialty Community Provider during 2018/19.

Delivery of the plan is overseen by the Integrated Commissioning Executive on behalf of the Dudley Health & Wellbeing Board.
