Completed application forms should be taken to your local Housing Office, the Dudley Council Plus office or posted to:

Make sure you answer all the questions on the form and sign the declaration. Insurance starts when the Council informs you that you have been accepted onto the scheme. The Council will write to you with details of your weekly insurance premiums and the date when you should start paying.

 

 

All material facts must be disclosed. Failure to do so could invalidate your policy. A material fact is one that is likely to influence the insurer in the acceptance and assessment of this application e.g. intended unoccupancy of the property or any criminal convictions (other than motoring offences). If you are in any doubt as to whether a fact is material, then it should be disclosed to the insurer. If any changes in circumstances arise during the period of insurance cover please provide your insurer with details.
