The Children (Performances and Activities (England) Regulations 2014 states that in certain circumstances, children taking part in entertainment must be supervised by a chaperone.

Children in entertainment and performances, including; TV or filming, theatre, sporting activities or modelling must be in the care of their legal guardian (usually a parent/carer) or an approved chaperone who is responsible for ensuring their health, safety and education needs are met at all times.

Become a chaperone

If you want to become a chaperone, you must apply to the council area where you live.  Any adult living in the Dudley borough may apply to Dudley Council to be registered as an Approved Chaperone. The Education Investigation Service undertakes the 'approval' of chaperones.

Before you apply

  • Allow up to two months before the first rehearsal or performance where you attend to be a chaperone as several checks have to be made during the application process.

How to apply

  • Please download and complete the PDF or Word version of the application form below. (Please ensure you complete all fields, as failure to do so may delay your application).
  • Submit your completed chaperone application form and any other relevant documentation using the 'register to become a chaperone' button below.
  • If you do not have a valid enhanced DBS Certificate you will also have to apply to the Disclosure and Barring Service for an Enhanced Certificate of Disclosure before approval as a chaperone.  We can help you with this.
  • You will also need to pass our Children Social Care records check to assess your suitability for the role.

Any approval is subject to Dudley Council being satisfied that you would be able to exercise proper care and control of the child/children, and that you will not be prevented from carrying out your duty as chaperone by other duties. Any breach of these conditions could lead to the approval being withdrawn.

Once approved

  • Chaperones are approved for three years
  • After three years you will need to re-register to remain on our approved list
