Exercise Referral Scheme

It is safe and appropriate for almost anyone to be physically active. Physical Activity forms part of the treatment pathway for many illnesses, including diabetes, hypertension and obesity.

The Exercise Referral service, offers the opportunity to access appropriate exercise settings, to help you increase your activity levels. This could be regardless of your age, fitness levels or medical history.

If you are concerned about your health and are currently inactive then book an appointment with your GP who will refer you to an appropriate leisure centre, Healthy Hub Park or Action Heart.

Who is this scheme for?

To access the scheme you will be referred by a health care professional. The scheme if for people who are sedentary & possessing one or more of the following conditions

  • Family history of coronary heart disease (CHD)
  • Diabetes
  • Overweight/obese
  • Hypertensive
  • Smoker
  • Depression
  • Anxiety/stress
  • High cholesterol
  • People who require more supervision
  • Inactive

If you have three or more conditions, you may be referred to Action Heart.

What are the benefits of becoming more active?

Being more active can help:

  • Make you feel more energetic and lively
  • Form part of the treatment pathway for many illnesses
  • Improve your health and body shape

  • Provide you with a sense of achievement
  • Help achieve and maintain a healthy weight

How much exercise do I need to do to keep healthy?

The Department of Health recommends the following:

  • Adults should aim to be active daily. Over a week, activity should add up to at least 150 minutes (2½ hours) of moderate intensity activity in bouts of 10 minutes or more – one way to approach this is to do 30 minutes on at least 5 days a week.
  • Alternatively, comparable benefits can be achieved through 75 minutes of vigorous intensity activity spread across the week or combinations of moderate and vigorous intensity activity.
  • Adults should also undertake physical activity to improve muscle strength on at least two days a week.
  • All adults should minimise the amount of time spent being sedentary (sitting) for extended periods.

Individual physical and mental capabilities should be considered when interpreting the guidelines.
