A medical application should only be completed in the following instances:
- If your current home is unsuitable for you or someone in your household, due to health or disability needs and you feel this could be improved by a change of accommodation or
- If you feel you or a member of your household will need adaptations such a level access shower, stairlift, vertical lift, or adaptations so you can use a wheelchair indoors
The Assessment Process
On submission of a medical application form you are agreeing to further assessment through the Housing Occupational Therapy Service. The assessment will determine if you need to move home or whether your requirements can be met in another way. For example; if you are unable to use the stairs in your house to reach the rooms or facilities you need.
In this case your assessment will determine if there are any adaptations or equipment options that may help you within your current home or if it would be better for you to move to ground floor accommodation or a property with a lift.
For example; if you do not have a carer living with you and you require substantial support or care from a friend or relative, who is unable to travel to you, to provide the support you need.
In this case your assessment will determine if there are any other options to support you within your current home or of a move to a property nearer to your support would be better.
For example; if you have a mental health condition that is adversely affected by your present accommodation.
In this case your assessment will consider whether your mental health would be helped by a change of accommodation.
Medical applications for any of the following reasons will not be assessed; they may be considered elsewhere within your housing need assessment or you will be directed to other services to assist you;
temporary need such as pregnancy, short term recovery after an operation or illness
overcrowding, problems with neighbours, anti social behaviour or harassment
damp or repairs
requests for extra care, sheltered or supported housing only.
If your initial housing assessment has placed you in band 1 or 2 you do not need to complete a medical application unless you need an adapted property. Band 1 or 2 is the same as or higher than the maximum medical award, therefore if you are already in this band, and you submit a medical application, you will only be assessed to see if an adapted property is needed.

Once you have received your housing options interview and bidding details, you will be contacted by our Housing Occupational Therapy Team to progress your medical application. Due to high demand for our services, it may be up to 14 weeks before we are able to contact you. We apologise for this delay to our usual timescale, please bear with us whilst we are working though our backlog and ensure you have considered the terms and conditions above, before applying.
