Designated nurse

The Designated Nurse provides health advice to young people up to the age of 21 years by working in collaboration with the Care Experience team. The support given varies and includes sign posting to other services and referrals if necessary.  An annual review health assessment is completed with you up to 18 years as legally required. In addition to this, nurses provide a health passport to care leavers around the time of their 18th birthday, with all their health information included such as childhood illnesses, immunisations details, birth details and so on.

Free prescriptions

If you are a care leaver up to the age of 25 and working or in full time Higher Education, you are entitled to free prescriptions. Please discuss with your PA who will be able to apply for this on your behalf.

Special Educational Needs and Disabilities (SEND) Local Offer

We have a dedicated section of our website that holds all of our information about how all children and young people with a special educational need or disability can live well in Dudley.

It is the SEND Local Offer.

Support if you are pregnant

If you are pregnant and aged under 25, the Family Nurse Partnership programme can support you through your pregnancy. This free and voluntary programme will support you to give your baby the best start in life.
