Dementia Gateway Service Privacy Notice

The Dementia Gateway Service is part of Dudley MBC and provides an advice and information service for people with Dementia. The service receives requests for contact from wide range of people including self referrals, GP’s and the other health professionals, Council workers and the police and local hospital.

Information we collect

The requests for our service include both contact details and information about the health and well-being of the person being referred for advice and information.

This may include your name and address, telephone number and email address, date of birth, your diagnosis of Dementia, as this is the Criteria for receiving the service and any other health conditions they feel would help us to know about, for when we make contact with you. They will also usually provide a name and telephone number of your Carer.

Please be reassured that your personal information will only be used to make contact with you or to deliver the service you need, or to meet a requirement.

What is the legal basis for us to process your data?

The processing of your information is necessary for compliance with the legal obligations contained in the following Act:

  • The Care Act 2014

The Care Act provides a national eligibility Criteria and a framework for care and support with regards to focusing on the needs and preferred outcomes of the person. This requires us to take into account information relevant to the person and their circumstances in order to promote and enable their well-being, to balance this along with the needs of people who are involved with them, and to protect individuals.

Individual consent is obtained for support with individual recommendations to or direct support tasks with an individual. Individual consent is also obtained for inclusion in taking  and storing images which capture service related activity.

How will we use the information we hold about you?

Your personal information will be treated as confidential. The information you provide to us about yourself and others, for example about your family members or carers, will be used for the purposes of the provision of social care, in accordance with Data Protection Law.
 
Where appropriate and lawful the Dementia Gateway Service may share this information with other professionals and providers, both within the council and externally to the council, for the purposes of care management.  This may include the council’s statutory obligation in relation to safeguarding vulnerable adults. 

The Dementia Gateway Service, will store your details securely and confidentially within the Adult Information system; and other relevant council systems. 

Dudley MBC may also use your information for the purpose of performing any of its statutory enforcement duties.  It will make any disclosures required by law and may also share your information with other bodies responsible for detecting/preventing crime including fraud or auditing/administering public funds.

We will not disclose your personal information to third parties for marketing purposes

What are your rights?

You have the right to request a copy of the data the Dementia Gateway Service holds in connection with the services we provide to you (known as a subject access request).

You also have the right to request restrictions on the information we hold about you request a rectification or erasure of that information, and object to personal data which the Dementia Gateway Service holds in connection with providing a service to you.

You also have a right to complain to the Information commissioner’s Office about the way in which the Dementia Gateway Service, processes your Data.

How long will we keep your information?

We will keep your information for as long as it is required by us or other regulatory bodies in order to comply with legal and regulatory requirements or for other operational reasons.  We can confirm that all data retained is kept on a secure system that is compliant with strict government security standards.
