The 18-40 and 40+ teams work with people who have been assessed and are appropriate for mental health community social care services, or who have already been referred to mental health services and also need social care support.

Who is eligible?

Adults aged 18 and over who live in Dudley borough, or are registered with a Dudley borough GP, who have a mental health diagnosis or a diagnosis of autism.

When a Care Act assessment is needed

People will be supported to access care and support when required, after a social care assessment of need has taken place. This support may include things like help at home, supported living, or even residential care. The team will ensure that the most appropriate funding is accessed to help fund these care and support needs, where possible.

What is on offer?

Support on offer from the teams includes help to access training, employment, volunteering, and other activities that suit each person’s needs and aspirations.

People will also be referred to other helpful services to support their health and wellbeing. These may include housing, benefits and health services, along with others.

The teams will always work in partnership with people, listening to their wants and needs and always listening to them and being led by them but ensuring safety for all.

They will ensure that the required statutory duties are met relating to carers and any other duties as declared by the Care Act 2014, Mental Health Act 1983 (amended by the 2007 Act), The Mental Capacity Act 2005, The Human Rights Act 1998, and any other related legislation.
